Come prepared to adopt by printing, reading and filling out all the adoption papers in advance:  Adoption Application You may scan and email this form back to us at sullivanshelter@gmail.com or bring the form with you to the shelter.

Another option is to fill out this form and submit it to us.  Then, when you come to the shelter, you will sign the adoption paperwork.
